哥本哈根大学的研究者通过研究,发现了机体中狭窄心脏瓣膜和血液中的一种特殊脂蛋白之间的关系,这或许可以帮助研究者开发治疗心脏瓣膜疾病的新型疗法。

2013年11月15日 讯 /生物谷BIOON/ --近日,来自哥本哈根大学的研究者通过研究,发现了机体中狭窄心脏瓣膜和血液中的一种特殊脂蛋白之间的关系,这或许可以帮助研究者开发治疗心脏瓣膜疾病的新型疗法,相关研究成果刊登于国际杂志Journal of the American College of Cardiology上。

主动脉瓣狭窄(AS)即主动脉瓣的开口变窄,其可以影响2%-7%的65岁以上的丹麦人的健康,而且这种疾病具有致命性,目前最有效的方法就是进行瓣膜外科手术,但是这种疾病会发生遗传。

研究者Anne Tybjarg-Hansen表示,基于对大量人群的研究调查,我们发现,血液中脂蛋白水平角较高的个体,其发生主动脉狭窄的风险较高;而在这些脂蛋白水平较高的个体中,就有10%的个体日后会发展发生相关的疾病。

同时研究者在文中揭示,发生主动脉狭窄的风险在那些出生时就携带有特殊LPA基因突变体的个体中是最高的,而LPA基因突变就意味着个体的脂蛋白在其一生中都将处于较高水平,LPA基因在丹麦个体机体的主动脉狭窄发展过程中起到了非常重要的作用。

研究者希望这项研究可以帮助科学家们开发抑制个体主动脉狭窄的新型疗法。Pia Rorbak Kamstrup博士表示,短期内我们希望这项研究可以帮助医药企业进行临床试验,来调查降低血液中的脂蛋白对机体发生主动脉狭窄以及冠心病的影响;此前研究显示,心肌梗塞和血液中脂蛋白之间存在一种因果关系。

Elevated Lipoprotein(a) and Risk of Aortic Valve Stenosis in the General Population

Objectives To determine whether elevated lipoprotein(a) levels and corresponding LPA risk genotypes (rs10455872, rs3798220, kringle IV type 2 repeat polymorphism) prospectively associate with increased risk of aortic valve stenosis (AVS). Background The etiologic basis of AVS is unclear. Recent data implicate an LPA genetic variant (rs10455872), associated with lipoprotein(a) levels, in calcific AVS. Methods We combined data from two prospective general population studies, the Copenhagen City Heart Study (1991-2011;N=10803) and the Copenhagen General Population Study (2003-2011;N=66877), following 77680 Danish participants for up to 20 years, during which time 454 were diagnosed with AVS. We conducted observational and genetic instrumental variable analyses in a Mendelian randomization study design. Results Elevated lipoprotein(a) levels associated with multivariable adjusted hazard ratios for AVS of 1.2(95% confidence interval:0.8-1.7) for 22nd to 66th percentile levels (4-15 mg/dL), 1.6(1.1-2.4) for 67th to 89th percentile levels (16-55 mg/dL), 2.0(1.2-3.4) for 90th to 95th percentile levels (56-74 mg/dL), and 2.9(1.8-4.9) for levels >95th percentile (>74 mg/dL), versus levels <22nd percentile (<4mg/dL) (trend, p<0.001). Lipoprotein(a) levels were elevated among carriers of rs10455872 and rs3798220 minor alleles, and of low number of KIV-2 repeats (trend, all p<0.001). Combining all genotypes, instrumental variable analysis yielded a genetic relative risk for AVS of 1.6(95% CI, 1.2-2.1) for a 10-fold lipoprotein(a) increase, comparable to the observational hazard ratio of 1.4(1.2-1.7) for a 10-fold increase in lipoprotein(a) plasma levels. Conclusion Elevated lipoprotein(a) levels and corresponding genotypes associated with increased risk of AVS in the general population, with levels >74 mg/dL predicting a 3-fold increased risk.
